Due: September 17, 2007 Click on the link below to download the 2007-2008 Employment Verification Form if you are in your 1st – 4th years of teaching. If you completed your 4th year of teaching last year, please go back a page and download the 2007-08 Final Employment Verification Form. 2007-2008 Graduate Employment Verification Form (You will need Adobe Acrobat Reader to download the application materials. If you do not have Adobe Acrobat Reader you may download it free by clicking here and following the instructions for download.) See instructions for filling out the 2007-2008 Graduate Employment Verification Form below. The Teaching Fellows Graduate Employment Verification Form must be completed each year after undergraduate graduation (whether employed as a teacher in a North Carolina public school or not) and returned to the Teaching Fellows office until you have fulfilled your four-year service obligation in a North Carolina public or North Carolina U.S. Government school and have affirmed your fourth and final year of teaching. This means you will receive a Final Graduate Employment Verification Form after you have completed four years of teaching service. For each year (defined as "a minimum of six full calendar months") of employment as a full-time licensed teacher, or in another educationally related professional capacity as defined by the Commission, (if you entered in the class of 1999 or before this means as a school counselor, school psychologist, school social worker, speech-language pathologist, or media specialist), one year of scholarship/loan, up to $6,500.00, will be removed from your account. While experiences in other positions are valuable and provide professional growth, they cannot be credited as service for repayment of the loan. Instructions for the 2007-2008 Graduate Employment Verification FormIf you have not fulfilled your four-year teaching service obligation, you must fill out the front of the form completely. If your form is missing any information if will be returned to you. If you are currently teaching in a NC public school system: Give the form to your school system personnel director at the central office of the school system for which you are currently employed to fill out the back and have it notarized. The form must be returned as soon as possible to the address on the second page of the form. Failure to return the form places the entire loan in jeopardy. If you leave your form with your personnel director to complete and mail, you should stress that it should be returned by the deadline. You should follow up with your personnel director to be sure he/she mailed your form. If you are NOT teaching in a NC public school system: You must attach a letter of explanation stating your reason for not teaching. If you are in graduate school, ALSO with the letter of explanation, you must attach a letter from the registrar, including specific area of study and graduation date. Do not send a transcript. Remember that graduate work must be related to the teaching license (i.e., NOT law, pre-med, pharmacy, engineering, or doctoral work). The form must be returned as soon as possible to the address on the back of the form. Failure to return the form places the entire loan in jeopardy. The Second and Final Notices for Final Employment Verification Forms will be mailed October 12, 2007. If your form is not received by that date then you and your surety will have been sent a Second and Final Notice. If you were employed in the previous school year, make sure to affirm the completion of a minimum of at least six (6) full calendar months for that school year. This section can be found on the front of the form at the bottom. Your teaching service credit for the previous school year will be reversed if this area is not filled out completely. The Commission may also begin the collections process if it determines that you do not intend to repay in teaching service or are in graduate school in a non-related area of study or working toward a PhD full-time.
